I understand that Cuisine by Nadine wishes to photograph & film my child, as identified as follows: SPECIFIED PURPOSES: From time to time during classes photos and film footage are taken and published on social media channels, and www.cuisinebynadine.co.uk, as well as for advertising. This footage may be used for a period of upto 36 months.

I confirm that I am the parent or guardian of the child. *

I grant permission to Cuisine by Nadine to: (i)Photograph and film my child (ii) to use the media material for the specified purposes as detailed.

I do not object to Cuisine by Nadine storing copies of the media material for the specified purposes or to it storing my contact details on its database in case it needs to contact me.

I understand that: Cuisine by Nadine will process my and the child’s personal data in accordance with its privacy policy, a copy of which is at http://www.cuisinebynadine.co.uk/privacy-policy/ *

I can object to Cuisine by Nadine using the media material and/or the child’s or my personal data by contacting Cuisine by Nadine. I acknowledge that it may not be possible to withdraw footage from circulation if it has been published prior to me objecting to its use. *

I understand that: This consent form and any dispute or claim arising out of or in connection with it shall be governed by and construed in accordance with the law of England and Wales, and that the courts of England and Wales shall have non-exclusive jurisdiction to settle any dispute or claim arising out of or in connection with this consent form. *
